You can't extract keys from a SIM. You need to buy a SIM that comes with the keys (usually by email). You can also potentially buy a programmable SIM and program in your own keys. You can't do any of this with a commercial SIM card.
There are several suppliers who can provide such SIMs, including these that I am aware of:

As for roaming, I don't know the details of how this works, but my assumption is that providers share the keys with each other as part of a roaming agreement. You definitely can't use a SIM without knowing its private key.
